Employee killed when pinned by turbo unit and cylinder

Employee #1 was performing maintenance on a turbo unit of a sand mixer. He disconnected the air cylinder rod from the gate on the turbo unit and pushed the rod back into the cylinder. When he loosened a bolt on the turbo cover, he triggered a microswitch to close the gate. The rod moved forward, pinning Employee #1 between the turbo and cylinder. He was killed.
