Employee struck and killed by flying cant

At approximately 7:35 a.m. on July 24, 1991, Employee #1 was standing approximately 20 feet behind a coworker who was straightening out the wood cants on conveyor lines #1 and #2 before the cants entered the gang rip saws. The coworker was operating the gang rip saw on line #1 when Employee #1 said a cant was hung up on the line. The coworker moved the conveyor's kickback fingers and toggled the roll direction switch to reverse, then back to OFF. A piece from a cant in the arbor blades shot out between the kickback fingers and struck Employee #1 in the side. He sustained internal injuries and died. The employees failed to follow the correct procedures for removing a cant from the gang rip saw.
