Employee severely injured in fall through skylight

At approximately 11:00 a.m. on September 12, 1991, Employee #1 was instructed to shovel gravel and pick up trash along the tie-in on a roof. At 12:15 p.m., everyone stopped for lunch and was told to return to work at 1:00 p.m. At approximately 12:35 p.m., Employee #1 was found on the concrete floor under a broken skylight opening. Apparently, he sat on the skylight cover, which broke, and fell 31 feet to the floor. The skylight opening was 7 ft 4 in. by 4 ft 4 in. with no supports underneath. The employee sustained severe head injuries and a broken arm. He was hospitalized in critical condition.
