Event: 11/29/1994

Employee dies when struck by falling steel culvert form

At 8:00 a.m. on November 29, 1994, Employee #1 drove a flatbed trailer to the storage yard at Dekalb Concrete Company. He exited the truck and waited for the steel culvert form to be loaded by a forklift onto the flatbed trailer. As soon as the culvert form was loaded, Employee #1 started securing the form to the trailer bed with chains. At 9:00 a.m., he climbed onto the truck tires and walked in front of the culvert form on his way to the back of the truck cab to get DOT signs. As Employee #1 stepped off the tires onto the trailer bed, the end jacket arm on the culvert form broke loose and struck him across the shoulders. It knocked him to the trailer bed, crushing his head. He died on the way to the hospital.
