Investigation Summary
On February 21, 1994, Employee #1, who was normally assigned to the column crew, was temporarily assigned to the deck crew. Employee #1 had been hit in the face by a shackle from a chain sling earlier in the day. At approximately 2:00 p.m., Employee #1 was getting some lumber from a bundle that was stacked on top of a 12 foot 6 inch scaffold without wearing fall protection. He was going to use the lumber to install formwork for a concrete roof. Employee #1 popped the first of the two bands that holds the bundle together and then popped the second band. He was struck by falling lumber and fell to the concrete deck. Employee #1 died the next day from head injuries.
