Investigation Summary

On or about April 11, 1996, Employee #1, a service technician HVAC Technical, Inc., arrived at Revcor Molded Products to replace the fan motor on a roof-mounted air conditioning system. He spoke with two Revcor workers while he placed a 28 ft extension ladder on the building. The two Revcor workers then went back inside the building, but in a very short time they heard a loud noise and came back out to investigate. They found Employee #1 lying on the ground, and called 911. He died of massive head injuries. Employee #1 had apparently secured the ladder to a rain gutter support, and had been returning to the ground to pick up his tools when the ladder slipped and he fell onto the concrete surface.
