Employee dies month after fall from stepladder

At approximately 7:15 a.m. on December 4, 1992, Employee #1 was cutting an 8 in. by 10 in. collar in a section of sheet metal duct located below a 12 ft high ceiling. To reach the section, Employee #1 was standing on a 10 ft wooden type I stepladder, manufactured by Lynn Ladder and Scaffolding Co., Inc., Lynn, MA. Employee #1 fell backward from the ladder and struck his back and head on the concrete floor. Employee #1 sustained a concussion; he was transported by ambulance to a hospital trauma unit and treated in the intensive care unit until he died on January 10, 1993. The investigation could not reveal how the ladder was used.
