Employee killed when run over by dump truck

At approximately 11:35 a.m. on July 8, 1994, Employee #1, a laborer, was standing at a freshly backfilled trench after installing a 20 foot length of 8 in. OD water main. An 18-yard dump truck, traveling in reverse, ran over Employee #1, who was standing with his back to the truck. He sustained multiple fractures and died.
