Investigation Summary
Employee #1 had been raised on the forks of an electric industrial truck in order to place boxes of magazines in storage racks in a warehouse. After placing a box in the upper rack, the industrial truck operator began to lower Employee #1 while simultaneously advancing the industrial truck. Employee #1 was facing the operator and holding onto the frame of the forks. The operator did not see a structural tie beam that obstructed passage of the elevated portion of the industrial truck. Employee #1 was struck by the beam. He complained and the operator lowered him. Employee #1 walked off the forks and collapsed. He was hospitalized and died 3 months later.
