Employee is crushed to death by falling X-ray machine

Employee #1 and two coworkers were attempting to load a mammograph X-ray machine that weighed approximately 1,300 pounds onto the tailgate of a rented truck. When the machine was nearly loaded, one coworker stood by the handle that raises and lowers the gate. Employee #1 and the other coworker then moved the machine fully onto the tailgate, and the coworker at the controls started to raise the gate. When the tailgate was approximately 3 to 3 1/2 feet high, the machine began to fall off. Employee #1 apparently tried to stop it from falling and was crushed to death when the machine fell on him. The coworker who had stayed at the tailgate was slightly injured. He was taken to the hospital and released after receiving first aid.
