Investigation Summary
Employee #1 and coworkers were stringing cable across a highway from an aerial truck when Employee #1 removed himself from the work area and sat against the wheel of the vehicle. When the vehicle was moved, the employee was run over and killed. Employee #1 suffered a fractured skull and chest. The employees on the vehicle did not know that Employee #1 was missing.
