Employee killed when struck by backing tractor

Employee #1 was returning from an empty case storage pile with an empty pallet jack. A yard tractor was backing into a bay to pick up an empty trailer. The tractor was equipped with a fifth wheel, which required that it be maneuvered back and forth in order to be properly aligned with the front of the trailer. Employee #1 was struck by the left rear wheel of the yard tractor and was killed.
