Employee killed after striking head in same-level fall

At approximately 10:00 p.m. on June 18, 1996, Employee #1 was cleaning up an ice caddy that had overturned in the middle of the aisle and had spilled ice and water on the floor. He slipped and fell, striking the back of his head on the tile floor. Employee #1 was transported to the hospital, where he died two days later.
