Employee dies in fall after slipping on frost patch on roof

At approximately 9:30 a.m. on December 17, 1993, Employee #1 was working on an 18 foot high roof. There was an invisible frost patch on the roof. Employee #1 slipped on the patch, lost his footing, and fell to the ground. He died in the fall.
