EMPLOYEE KILLED BY FALLING EQUIPMENT
Employee #1 was guiding a portable blender full of flake down a monorail system. The portable blender went through an open switch, crashed through the safety stops, and fell to the floor. The blender fell on and crushed Employee #1 inflicting multiple injuries that resulted in his death.
