Employee injured in 30 ft fall into excavation

Employee #1 was handling the tag line on a clam bucket being used to clean out the bottom of an excavation. As he walked along the excavation's edge, he slipped and fell into it. The employee slid down the side of the excavation, striking his head against either the frozen ground or the form work at the bottom of the excavation. The excavation was 30 to 32 ft deep, and the side walls were sloped at a 35 degree angle. The side walls and the work site were covered with a thin layer of snow. The edge of the excavation was clear of snow and ice.
