Investigation Summary
Employee #1 was performing his assigned duties of cleaning and disinfecting a pulp mixer. As part of the procedure, Employee #1 turned on the pulp mixer and removed all guards protecting the rotating screw paddles. When Employee #1 stepped onto a mixer to reach another, his foot slipped and caught in the rotating screw paddle. His leg was later amputated below the knee.
