Employee killed by collapsing concrete septic tank

Employee #1 was just inside a circular concrete septic tank that had been lifted out of the ground and laid on its side. He was removing a lifting rig that had been used to pull the tank out of the ground. The tank, which was 9 1/2 ft in diameter and 10 ft long, had been in the ground for approximately 20 years and was being removed for installation at another location. The concrete was several inches thick and weighed approximately 10 to 15 tons. The tank suddenly and completely collapsed into sections with Employee #1 inside. He was crushed and killed.
