Investigation Summary
A 2,700 lb motor control box was housed in a wooden packing crate (16 1/2 ft long, 7 ft 1 in. high, and 20 in. wide) in the back of a covered moving van. Due to its length, the package rested lengthwise against a sidewall. The operator of a powered industrial forklift was off loading the package but the package's position prevented him from picking it up near its center. Instead, the operator picked the 16 1/2 ft long package up lengthwise, on a lifting fork that was just under 4 ft in length. As he "snaked" the vehicle out of the covered trailer, the package began to tip. When Employee #1, a ground tender, attempted to manually stabilize the package, it continued to fall from its precarious position, crushing and killing him.
