Investigation Summary
Employee #1 was using an overhead crane and scissor lift clamp to load 4 by 8 ft slabs of marble onto an A-frame rack that was mounted on a truck. Employee #1 set the first sheet of marble onto the rack and then picked up two more slabs of marble. When he set down the slabs to position them, the clamp did not lock in the open position. Employee #1 then reached up with one hand to lock the clamp while balancing the slabs with his other hand. In the process, the slabs shifted and knocked the first slab off the rack and into the other two. Employee #1 was knocked off the truck and pinned against the wall by the marble slabs. He was crushed and killed.
