Employee killed in fall from ladder

Employee #1 was using an extension ladder to put up fascia board at a new home construction site. The ladder's feet were placed on a mound of dirt that had been created by dirt moved away from the basement windows. Employee #1 climbed approximately 20 feet to the top of the ladder He fell, landing on the dirt adjacent to the window, and was killed. The ladder was not tied off at the roof and the foot of the ladder was unstable.
