Employee killed in 21 ft fall from roof

Employee #1, a framing contractor, was installing OSB roof decking with the help of two subcontractors. Employee #1 was walking back and forth on a 5:12 pitch roof peak, cutting and stocking OSB for one of the subcontractors to nail down. Employee #1 fell 21 feet from the roof peak to the decked part of the roof, then to the ground, and died from multiple trauma. None of the workers was using fall protection.
