Investigation Summary
On October 10, 2001, an employee of Foundry Inc. Dba Sealers, Inc. was emptying an industrial mixer that was used for combining 1500 pounds of rubber compound materials. The mixer was turned off and tilted allowing the material be emptied. The employee turned the mixer on and reached into the rotating blades to knock off a piece of material when he was pulled into the mixer. A coworker on a forktruck jumped off and turned the machine off. The victim was pulled through the mixer more than once before the machine was turned off. The employee died of cuts and lacerations.
