Investigation Summary
At 1:45 p.m. on November 25th, 2011, Employee #1 was attempting to off-load a Volvo semi-truck from a heavy-duty commercial tow truck at a Volvo dealership. Employee #1 had lifted the Volvo tractor vertically with the tow truck's boom, and placed the front tires on wooden blocks. Employee #1 then lowered the tow truck's boom and climbed underneath the Volvo tractor to release the safety chain from the front axle. The Volvo tractor rolled forward off the wooden blocks and crushed the tow truck driver. There were no witnesses. Employee #1 was pronounced dead at the scene.
