Investigation Summary
At approximately 9:15 a.m. on April 22, 1995, a tractor trailer loaded with scrap shavings and grinding mix backed up parallel to a barge that was sitting at a boat dock/terminal. After the crane operator took some scrap from the rear of the tractor trailer, the driver of the tractor lifted the trailer approximately 18 to 20 ft high. The trailer tilted to the left and tipped over onto the barge. Employee #1, a safety spotter, who was standing on the catwalk of the barge, saw the trailer tipping and ran to get out of the way of the falling trailer. He apparently slipped and fell, and his head was crushed by the trailer against the barge. Employee #1 died.
