Investigation Summary
On September 6, 1994, Employee #1 was aboard a container vessel called the President Harrison at terminal 5, Seattle, WA. He and several coworkers were preparing to load provisions into the focsle hatch using a 50 LT shore-based container crane with rigging hung from one corner of a standard 20 ft container spreader beam. Employee #1, who was in radio contact with the crane operator, was on the main deck guiding the load into the hold. As the load was lowered into the hatch, Employee #1 apparently peered over the railing surrounding the hatch to spot the load and the spreader beam crushed his skull against the top of the railing, killing him instantly. Evidence gathered during the investigation shows that the crane did not malfunction but that the rigging used was almost 4 feet too short for the intended task, creating a pinch point. A general duty violation for the use of short rigging is proposed.
