Investigation Summary
At approximately 9:30 a.m. on June 22, 1992, Employee # 2 was moving a 42 ft long by 11 ft 11 in. high grain trailer, serial #83374, into a loading bay on the south end of a plant. He was backing into the building when he struck and ran over Employee #1. Employee #1 suffered severe internal injuries and was hospitalized. He later died. The truck was a model J3000R, serial # 791.
