Event: 05/14/1997
Employee killed when caught in impeller blades of turbine

Employee #1 and four maintenance coworkers were removing a 200 hp electric motor from a turbine used to generate waves for a wave pool. Another Wavetek, Inc., turbine, approximately 15 ft away, was running with unguarded impeller blades. Employee #1 apparently walked over to it and either tripped or slipped and fell into the machine, or he got too close and the suction of the machine, which pulled approximately 80,000 cfm, drew him into the impeller blades. Employee #1's body from mid-chest up was amputated by the machine, and he died instantaneously. There were no witnesses to the accident.
