Employee killed in fall when guardrail anchor breaks loose

Employee #1, a carpenter, was working on the third floor of a building under construction. For some reason, he leaned heavily on the top chain of a chain guardrail system while looking to the ground. The chain broke loose and he fell head first 28 ft to the ground. Employee #1 was killed. An anchor of the top chain pulled out on one end where it was installed in a concrete block, but it did not break. A chain in the middle of the guardrail remained in place.
