Employee killed when crushed in cardboard compactor

At 7:00 a.m. on January 20, 1995, Employee #1 was assigned to do lawn maintenance. At about 8:30 a.m., his supervisor noticed that he was missing. After a search, Employee #1's body was discovered inside a paper box compactor/baler. The baler is equipped with interlocks; activation of the machine is only possible if both the lower door and slide gate are closed. The slide gate on the compactor was missing, leaving a 6 1/2 in. by 22 1/2 in. opening that was large enough for someone to be able to reach out and activate the compactor from the inside.
