Investigation Summary
Employee #1 and five coworkers were assigned to re-roof a newly constructed gymnasium. They began their early morning duties by placing their materials at strategic locations on the roof. Three employees were assigned to unload a cart of drywall onto the roof deck next to an unguarded skylight. Employee #1 was walking backward with a sheet of drywall when he stumbled on the base of the skylight and fell through the opening onto a concrete floor approximately 30 ft below. Employee #1 died days later at the local hospital.
