Investigation Summary
Just after lunch on January 28, 1995, Employees #1 and #2 went out onto lagoon #3 to check a problem with a transfer pump and determined that the cause was a bad motor. Around 2:00 p.m., a pontoon was brought from another lagoon to lagoon #3. A new motor, weighing 480 lb, was tied to the right pontoon at the front. A full set of oxygen and acetylene bottles was tied to the rear, in preparation for burning the nuts off the motor. Around 3:15 p.m., Employees #1 and #2 got on the boat and headed out to change the motor. At around 4:10 p.m., an employee left the plant and saw the men working on the motor. Around 4:30 p.m., another employee left the plant and noticed that the pontoon had capsized; he returned to the plant and contacted management to initiate a rescue. It took until around 4:00 p.m. the next day to recover the bodies. Employees #1 and #2 had drowned. They were not wearing life vests.
