Investigation Summary
At approximately 10:00 a.m. on December 6, 1995, Employee #1 was instructed by his supervisor to ferry loaded freight dollies outside using a GPS Promatic tire truck. Employee #1 dismounted the industrial truck leaving the blades up and with the load partially attached so that he could jack up the dolly and hook it up with the other dolly. The supervisor found Employee #1 straddled between the two dollies and slumped over the blades of the forklift. The tongue, which is on hinges, must have flopped up and struck Employee #1 in the face because the dolly was now on the ground. He was killed when he was pinned by the forklift blades. There were no witnesses to the accident.
