Investigation Summary
Employee #1 was welding an extension onto a 12 inch "H" batter pile. He was standing approximately 30 feet from a vertical 38 foot "H" pile that was being cut off. The pile was to be only partially cut, but it was cut all the way through and fell, striking Employee #1's upper body and killing him. The batter pile had not been secured, nor had other operations been suspended within 76 feet of the pile being cut.
