One employee killed, one injured when scissor lift overturns

At 9:00 a.m. on March 23, 1995, Employee #1 was operating a scissor lift to the catwalk area of a theater. Employee #2 was inside the lift. The lift turned over on its side, injuring both employees. Employee #1 died on May 12, 1995.
