Employee killed when struck by length of steel pipe

Employee #1 and two coworkers were loading trucks with 14 in. diameter steel pipe from an outside stock area. The pipes, which had 1/4 in. walls and were 44 ft to 64 ft in length, were stacked six pipes high. The pipes were blocked on each end and each layer of pipe had rough 2 by 4's between them. Employee #1 was on the top of the stack of pipes and rolled a length of pipe about 61 ft long to the front of the stack where the Pettibone model 204C could reach it. When Employee #1 got in front of the pipe it rolled off the stack and struck him. He was killed.
