Employee falls from scaffold and dies

Employee #1 and another worker were moving a mobile scaffold working platform from an elevation of 46.5 inches from the floor to 112 inches. They moved one of the two metal planks and were positioning it in place in order to raise the second plank when Employee #1 lost his balance and fell backwards. He fell off the lower platform striking his head on equipment adjacent to the scaffold and died.
