Event: 08/20/1998
Employee dies in fall from roof onto truck

At approximately 11:30 p.m. on August 20, 1998, Employee #1, in preparation for going home, was clearing trash and wooden pallets from a new roof that his company was installing. Trash and pallets were thrown over the side of the roof into a parked truck. Employee #1 was throwing a pallet when he lost his balance and fell 15 ft onto the truck. He suffered blunt torso impact, including a lacerated chest and spleen, and fractured ribs and collarbone. Employee #1 was transported to the hospital, where he later died of his injuries. Adequate fall protection was not provided.
