Investigation Summary
Several employees were on scaffold platforms framing the exterior of a condominium building under construction. The platform surfaces were constructed by using two 2-in. by 12-in. boards nailed together. The platforms were placed through windows at the east side of the building and resting on a 2-in.by-4-in. board nailed on the west side on the building wall. The scaffold platforms were not provided with guardrails and no other fall protection was utilized. Employee #1 was on a 22-ft high platform installing hurricane straps. While trying to move the plank, Employee #1 fell approximately 9 ft and landed on a platform 13 ft above the ground. This platform was erected the same way as the one he had been standing on. Employee #1 then fell through a window opening in front of the lower platform and into the building entryway. Employee #1 was killed.
