EMPLOYEE DIED AS TRACTOR WAS BACKING UP

Employee #1 died as the tractor was backing up. The operation involved extending a structural steel stretch trailer to accommodate longer length steel members to be hauled. Employee #1 was at the side of the frame of the trailer and had already made the proper connections. He told the spotter to tell the driver of the tractor to back up. After the driver backed the tractor up, the spotter then turned back and noticed that Employee #1 had moved again under the frame. This time, he was found dead crushed between segments of the sliding trailer.
