Employee killed in fall with elevated platform

At approximately 4:30 p.m. on February 4, 2000, Employee #1 was working from a personnel platform situated on the forks of a Caterpillar Telehandler, serial #SWM02200. He was touching up the stucco work on the front of a building under construction when the platform came off the forks. Employee #1 fell approximately 8 ft to the ground and died as a result of head injuries. The platform was not secured to the forks.
