One employee killed, one injured when aerial lift overturns

Between 7:30 a.m. and 8:00 a.m., on March 29, 1994, Employee #1 and Employee #2, the lift operator, were standing on the platform of a self-propelled aerial lift installing the top sections of sheetrock near the ceiling in auditorium #3. The lift was on an incline, and the lift's platform deck was elevated 18 ft 8 in. above the concrete floor and extended out 3 ft. They had measured and prepared 5 1/2 boards of sheetrock for installation when the lift toppled over toward the southwest corner of the auditorium. Employee #1 fell 20 ft and was killed; Employee #2 fell and sustained serious injuries.
