Investigation Summary
Employee #1 was operating an extruder that was making three wires into a cable by coating them with PVC. Two of the wires were being fed from axle-mounted spools and the third wire was being fed from a vertical spindle sitting on the floor. The third wire was being uncoiled from above, over a sheave that was mounted on a manual jack stacker. The jack was not secured to the floor. When the wire got hung up, the jack overturned and hit Employee #1, who was attempting to prevent the jack from overturning. Employee #1 died from his injuries.
