Employee killed in trench cave-in

At approximately 1:55 p.m. on June 1, 1993, Employee #1 was removing soil from a sewer tap in a trench that was 20 feet long by 30 inches wide by 12 feet deep. The walls were vertical and unshored. Employee #1 had been in the trench approximately 10 minutes when the north wall caved in, covering him to his chest. The employer had started removing soil from the north wall in an attempt to save Employee #1 when the south wall caved in, completely covering Employee #1 with soil. He was killed.
