Investigation Summary
On February 17, 1995, Employee #1, of the case dock crew, rotated up to the second floor of the plant to monitor the material handling system that moves the milk crates through the area. One of his jobs was to free up any milk crates that frequently jammed the system. The employee was authorized to turn the machine off, but not to lockout a machine or component of the material handling system. At approximately 2:50 p.m., the automatic stacker that loads the overhead conveyor apparently had jammed. Employee #1 attempted to unjam the machine while it was still on; the milk crate loading ram cycled, striking and fatally injuring Employee #1.
