Employee thrown to ground in forklift overturn

Employee #1 and a forklift operator were attempting to adjust a light fixture on top of a light pole in a parking lot. They placed a pallet on the forks of a telescoping boom forklift. Employee #1, who was on the pallet, put on a safety belt and tied off to the load backrest extension. They drove across the parking lot. When they reached the light pole, the operator raised Employee #1 more than 30 feet into the air. When the boom was almost fully raised and fully extended, the operator saw that Employee #1 could not reach the light fixture. To bring him closer to the light, the operator turned the wheels hard to the left and pulled forward a few inches. The forklift overturned to the right. Employee #1 was thrown to the concrete on the end of his safety belt; he was hospitalized.
