Investigation Summary
At approximately 4:00 p.m. on December 22, 1993, Employee #1 was discovered lying face down on the floor of the meat department's back room by a coworker. His face was blue and the coworker summoned management. An employee with CPR training arrived in the area and began performing first aid on Employee #1 while waiting for the ambulance to arrive. Employee #1 was transported to an area hospital. He died from an apparent heart attack.
