Investigation Summary

Investigation Nr: 14489132
Event: 07/06/1994
Employee dies of apparent heart attack

Employee #1, 68 years old, was an unarmed security guard working for a financial banking firm. He did not work on July 4th. On July 5th, he worked in the Brooklyn location. On July 6th, he was assigned to work in the Manhattan location where he works on occasion, depending on need. He arrived in the third floor main security office at approximately 6:25 a.m., to begin work at 7:00 a.m. His supervisor offered him coffee but he indicated that he had just bought a cup from outside and was preparing to drink it. Employee #1 then slid down the wall into a sitting position; he did not even taste the coffee. He tried to stand by himself and said "What the hell happened?" He then fell backward to the floor, losing bladder control and experiencing intermittent, labored breathing. The supervisor called Firewatch and EMS. Firewatch began CPR at 6:28 a.m. EMS medic #8670 arrived at 6:40 a.m. Employee #1 was transported to Bellevue Hospital at 7:05 a.m., where he was pronounced dead on arrival at 7:30 a.m.
