Employee killed in 5 ft fall from platform

Employee #1 and a coworker had been raised approximately 5 ft by a forklift truck and stepped off the truck's platform onto a metal shelf. As Employee #1 and the coworker lifted a metal bar to place it onto the forks, Employee #1 fell 5 ft to the concrete surface. He died approximately 2 weeks later from the injuries he sustained in the fall.
