Investigation Summary
Employee #1 worked for a company that installs septic tanks. After many days of rain and several attempts to install two septic tanks, the company finally was able to install the tanks in the ground. Due to the rain the sandy soil surrounding the second septic tank, which was approximately 5 ft deep in the soil, was muddy and fluid. When Employee #1 went inside the second tank to seal any openings that were leaking, the east side of the tank caved in, trapping Employee #1 inside and engulfing him with the surrounding muddy soil. Employee #1 was killed.
